How it works

How membership works, stage by stage.

VHN membership moves through four stages: a dedicated case manager, physical and mental health evaluations by independently licensed professionals, delivery of a complete medical record, and a courtesy introduction to an independent VA-accredited representative. Every stage has one owner and one deliverable. The record belongs to you.

Four stages. One standard.

Meet your case manager

An assigned case manager becomes your single point of contact from the first conversation. They coordinate your appointments, keep your records in order, and stay with you from start to finish, not just until your file is delivered.

Meet your team

You are introduced to your evaluation team: an independently licensed physician, an independently licensed mental health professional, and, as a courtesy of membership, a vetted independent VA-accredited representative. The representative is not part of VHN, operates under their own authority, and you are always free to work with any representative or VSO you choose.

Your medical record

Your providers produce ICD-10 coded clinical documentation, functional impact documentation, and a 12-month care plan reflecting their independent professional judgment. The complete record is organized and delivered in submission-ready format. It belongs to you.

Care established, claim submitted

Your care plan is in hand and your record is complete. If you choose to pursue a claim, your independent accredited representative submits it under their own authority. VHN built the medical foundation. What gets built on top of it is between you, your representative, and the VA.

What happens at each stage.

What happens in the clinical intake?

A former military medic, corpsman, or line medic conducts your clinical health intake. You cover your health history and your service history in plain terms. Your case manager uses that intake to prepare your evaluation team: each provider receives an intake packet and your service history before your first appointment.

Who performs the medical evaluations?

Two independently licensed professionals: a physician for your physical health and a mental health professional for your mental health. Each evaluates your full picture within their own scope and documents what they find. Their judgment is their own. There is no mechanism at VHN to direct it.

What does the documentation contain?

ICD-10 coded diagnoses, coded to the specificity the evidence supports. Functional impact documentation stating how each condition limits work, sleep, and daily activity. A 12-month care plan written in the provider's own clinical voice. The record is organized and delivered in submission-ready format.

What happens once your record is delivered?

The record belongs to you. As a courtesy of membership, VHN introduces you to a vetted independent VA-accredited representative, and you remain free to take your file to any representative or free VSO instead. If you pursue a claim, your representative files it under their own authority.

The pace

As fast as you are ready to move.

The pace is set by you and by the work. Your case manager coordinates three professionals around your schedule: a physician for your physical health, a mental health professional for your mental health, and the independent accredited representative handling your claim. Some steps sit outside VHN's control, like the VA recording the power of attorney you establish with your representative. And if you meet your doctor in person, the route VHN recommends over telehealth, the appointment happens when you can get there. VHN moves as fast as humanly possible. It will not trade thoroughness for speed, because the record is about your health.
