Case management is the coordination layer of healthcare: one accountable person or team managing appointments, records, referrals, and communication so that the clinical work happens on schedule and the documentation ends up complete and in one place.

The need for it grows with complexity. A person seeing multiple clinicians, across physical and mental health, accumulates records in multiple systems, appointment chains that depend on each other, and administrative tasks that no single clinician owns. Without coordination, the gaps show up as missed appointments, records that never made it into the file, and documentation that contradicts itself because no one read it as a whole.

A case manager's output is a process that completes: evaluations that happen in the right order, records gathered from every source, a file that is organized and legible. In any setting where the medical record will be read by a third party, that organization is part of what makes the record usable.
