How to Apply for CHAMPVA

If the applicant has any other health insurance, including Medicare, a second form goes with it: VA Form 10-7959c, which VA titles CHAMPVA Other Health Insurance (OHI) Certification, current revision March 2025. The same form is used later to report a change in other insurance, which is the part that catches people out at 65 when Medicare arrives.

A third number belongs to a different job entirely. VA Form 10-7959a, the CHAMPVA Claim Form, is filed after care to seek reimbursement. It is not part of applying, and it carries a deadline the other two do not: under 38 CFR 17.276, a claim for medical services must be filed no later than one year after the date of service, or one year after discharge for inpatient care.

Anyone qualifying through the caregiver route in 38 CFR 17.271(a)(4). That designation is made by the Program of Comprehensive Assistance for Family Caregivers, not by anything sent to CHAMPVA, so the paperwork that matters is the caregiver program's.

VA states what happens next in one sentence: if VA determines that the person qualifies, VA enrolls them automatically and nothing else is needed. So the application to make is the caregiver program application, and 10-10D is the wrong envelope.

Two routes VA publishes in full, and it treats them as equivalent. Online through VA.gov, or by mail to VHA Office of Community Care, CHAMPVA Eligibility, PO Box 137, Spring City, PA 19475. VA also names fax as an option without printing the number on the page, so anyone wanting to fax should get the current number from the call center on 800-733-8387 rather than from a third-party site.

It depends on the situation, and VA organizes the list that way rather than as a single checklist.

Where there is other health insurance, including Medicare: a copy of the front and back of the insurance card or Medicare card, submitted with VA Form 10-7959c. A Medicare Part D card is copied as well if there is one. Where the applicant is 65 or older and does not qualify for Medicare, VA asks instead for a notice of disallowance from the Social Security Administration, which is the document confirming that no Medicare entitlement exists under anyone's Social Security number.

For a spouse or surviving spouse, VA lists a marriage certificate, a certificate of civil union, or a common-law marriage affidavit as optional documents that speed processing. For a surviving spouse who remarried before 55 and whose remarriage has since ended, the optional document is a divorce decree, an annulment decree or a death certificate.

For a child, a birth certificate. Adoption papers where the child is adopted. Proof of the marriage between the veteran sponsor and the child's other parent where the child is a stepchild. For a child aged 18 to 23 in school, a school certification letter proving enrollment, which then has to be recertified once a year. For a child who became permanently unable to support themselves before 18, a disability rating letter for the child, which VA sometimes calls a helpless child rating and which a VA regional office can explain how to obtain.

A newborn needs two things done before the application can work at all. First a Social Security number from the Social Security Administration, then the child added as a dependent of the veteran sponsor through a VA regional office. VA states that it cannot pay medical claims for a qualifying newborn until the child is enrolled in CHAMPVA.

VA does not publish a processing time. It is not on the CHAMPVA program page, it is not on the form page, and it is not in the CHAMPVA Guidebook. That is worth stating plainly rather than replacing with an estimate, because any number circulating for this has no government source under it.

What VA does describe is the outcome. On approval, VA mails a welcome letter and a CHAMPVA identification card, and the Guidebook can be read online or requested as a printed copy on 800-733-8387.

The obstacle is more often the underlying finding than the paperwork. CHAMPVA asks for a permanent and total adjudication, which is not the same thing as a 100 percent rating, and that distinction is worth settling before an envelope goes anywhere. A recognized veterans service organization will read the decision letter and say which finding is in it, at no charge.

Two things need keeping current, and both are the beneficiary's job rather than VA's. Other health insurance is reported on VA Form 10-7959c whenever it changes, which includes the first Medicare card arriving. School enrollment for a child aged 18 to 23 is recertified annually.

Claims are the other half. Where a provider bills VA directly, nothing is needed. Where a provider does not, the family pays and then files VA Form 10-7959a, within one year of the date of service under 38 CFR 17.276(a). The same section allows a written request for an exception where there was good cause for filing late, reviewed case by case, and gives a worked example: a delay caused by a primary insurer taking time to adjudicate first, where the delay is not attributable to the beneficiary. It also states what will not work, which is a delay caused by a provider's billing procedures.
