Diagnostic codes
VA Diagnostic Code Index
A diagnostic code is the four-digit number VA uses to identify a condition in the rating schedule at 38 CFR Part 4. The code determines which criteria apply and therefore which severity thresholds a rating turns on. Conditions without their own code are rated by analogy under 38 CFR 4.20.
Codes read from 38 CFR Part 4 on 2026-08-21, using the eCFR edition current at 2026-08-01. 727 codes listed. Last updated 2026-08-23.
What is the number on my decision letter?
It is a diagnostic code. Four digits, assigned under 38 CFR 4.27, and its job is to point at a set of rating criteria. The first two digits say which body system, and the code as a whole says which criteria were applied to the condition.
A code with a hyphen in it is doing something more specific. Under 4.27 an unlisted condition rated by analogy gets a built-up number: the body system, then 99, then a hyphen, then the code whose criteria were actually used. So a hyphenated code is a decision showing its own working.
The code does not decide the percentage. It decides which criteria the percentage is read from, and the record decides which of those criteria the evidence meets.
An accredited Veterans Service Organization representative can look at a decision letter with you and explain which code was applied and why, at no charge.
Conditions covered in detail
These are the conditions people search for by name often enough to be worth a page of their own, with the criteria set out beside the regulation they come from. Every other code in the schedule is a row in the table below, which is the honest place for it.
The whole schedule
Every diagnostic code in Part 4, grouped by body system. Names are the schedule's own, shortened where they run long.
Eight codes read "no longer in the rating schedule". Part 4 prints them as removed, which is what happens when a body system is rewritten and a code is not carried into the new version. They stay in this table because an older decision letter can still have one printed on it, and a search that returned nothing would leave the person holding that letter unable to tell whether they had mistyped. What a removal means for a rating already assigned is a question for the section's amendment history and for an accredited representative, who can read the letter with you at no charge.
Type a four-digit code from a decision letter, or the name of a condition. The full table is below either way, grouped by body system.

Why do some codes have a page and most do not?
Because most of them have nothing to add. A page exists here only where enough people look the condition up by name to show that the question is being asked, and where there is something specific to say about the criteria beyond restating them. Everything else is a row in the table above.
The threshold was measured rather than guessed. It is 100 searches a month for the condition across every phrasing people use, deduplicated so that four ways of asking the same question count once. Nothing was written up to reach it.
Diagnostic code numbers themselves are not a page here, and that is also a measurement. Across the whole pattern of people searching for a code by its number, nine terms a month clear even a low threshold. The table answers that question in one place, which is the right shape for it.
See every level beside the regulation it comes from
The rating threshold cheatsheet shows every level for a condition in one view, plain English on the left and the regulation itself on the right, in a layout built to print.
It is free and it never says which level anyone meets. It describes what each one requires and leaves the reading to the person whose record it is.

Reviewed on a on-regulation-change cycle by Brayden Marley, who is accountable for what is on this page. Part 4 body systems are amended on a rolling schedule. A code whose criteria were rewritten leaves every page built on the old text factually wrong.
