Continuity of care

Continuity of care is an ongoing relationship between a patient and a clinician or care team: the same people see the patient over time, know the history, and document it consistently in one place.

Clinically, continuity changes what a visit can accomplish. A clinician who has followed a condition for a year writes different notes than one meeting the patient once. Documentation gains the same way: a record built through continuous care shows chronology, treatment response, and persistence, with fewer gaps and contradictions than a file assembled from one-off visits across unrelated providers.

For veterans, continuity can come from the VA, from conventionally insured private care, or from membership models like direct primary care. The source matters less than the pattern. Consistent documentation over time is what makes a medical history legible to any later reader, including a benefits adjudicator.
